Uninsured Service Fees

Some services are not covered by BC MSP and therefore have an associated fee. The new fees will be in effect as of Nov 1, 2025

Common form fees:

General form fee - $50 (short letter or note from your practitioner, EI form etc)

Insurance forms - $50 - $250 (depending on the amount of pages, and length of time your practitioner will need to complete the form)

Driver’s Medical Exam form (note these require a booked appointment with your practitioner and there is a fee for the exam and completion of the form)

Blue border form- $175

Yellow border form- $250

Disability tax credit - $250

Staff are unable to waive any fee set by your practitioner. Please check the list of uninsured services prior to booking your appointment.

Costs are accurate as per Oct 15, 2025 and may be updated at any time; enquire with office staff for most updated costs

No show / missed appointments / late cancellations

Our office will charge for any pre-booked as well as same day - phone call/virtual or in person visits that are missed or cancelled with less than 24h notice for our Practitioners (physicians and nurse practitioners) and RNs. This will be strictly enforced.

Monday appointments should be cancelled by the Friday prior.
